Register with RecTrac

Required Medical Information
If you or your participant has a medical condition, allergy, or support need that staff should be aware of, please provide this information during registration.
For participants who require emergency medication, please provide medication details, including dosage, dispensing/administration instructions, and when the medication should be given.
If necessary medical information is not provided, program participation will be suspended until staff have the information needed to provide appropriate support.
For camp programs, all required documentation must be completed and submitted through ePact no later than 7 days before the first day of camp. Incomplete documentation may result in the camper being unable to attend until all required forms have been submitted and reviewed.
